Request Samples Online

To complete your sample request:

1. Click the “Request Samples” link below
2. You will be taken to our secure DocuSign^® portal where you will enter the healthcare practitioner’s name and email to be directed to the sample request form
3. Enter all information that is required under compliance guidelines to receive samples, add your signature at the bottom of the form, and then click “finish”.
